- Memiliki Ijazah /STTB
a. MAN/ MAS Semua Jurusan
b. SMA/ SMK Semua Jurusan - Membayar Uang Pendaftaran Rp. 200.000,-
- Mengisi formulir pendaftaran yang telah disediakan
- Melampirkan dokumen sebagai berikut:
a. Fotocopy Ijazah SLTA dan nilai UAN/SKHU yang dilegalisir oleh pejabat berwenang masing-masing2 lembar
b. Fotocopy Kartu Keluarga (KK) 1 Lembar
c. Melampirkan pas photo hitam putih ukuran 3 x 4 = 4 lembar
d. Bagi ASN (Aparatur Sipil Negara) / Polisi, atau Instansi yang sejenisnya, harus ada surat izin dari pejabat erwenang
e. Semua berkas dimasukan dalam map yan diserahkan oleh Panitia Penerimaan Mahasiswa Baru
