B. Syarat Pendaftaran:
- Mengisi formulir pendaftaran yang telah disediakan
- Melampirkan Ijazah SLTA dan nilai UAN/SKHU yang dilegalisir oleh pejabat berwenang masing-masing 2 lembar
- Melampirkan pas photo hitam putih ukuran 3 x 4 = 4 lembar
- Bagi ASN (Aparatur Sipil Negara) / Polisi, atau Instansi yang sejenisnya, harus ada surat izin dari pejabat berwenang
- Melampirkan fotocopy Kartu KIP/ PKH sebanyak 1 Lembar (bila ada)
- Membayar uang pendaftaran sebesar Rp. 200.000
